Roles and Responsibilities - Employer

 

Interpreted Requirements

§          Employers shall:

o         Provide adequate and timely resources necessary to support their managed fall protection program.

o         Appoint a program administrator with the requisite knowledge, experience, training, and authority to direct the Employer's fall protection program.

o         Identify fall hazards and implement procedures for eliminating or controlling every existing or new fall hazard.

o         Develop and maintain written fall protection procedures for every location where an active fall protection system is used to control a fall hazard.

 

§          For Authorized Persons, Employers shall:

o         Inform each individual about any foreseeable fall hazard before exposure to the hazard.

o         Provide continuous fall protection at Employer-owned, or operated, locations or verify continuous fall protection at non-Employer-owned, or operated, locations.

o         Provide the knowledge and training necessary to operate any active fall protection systems used.

o         Supervise all persons who are exposed to fall hazards.

o         Verify that fall protection procedures are understood and followed.

Roles and Responsibilities - Program Administrator

§          The Program Administrator shall:

o         Develop and implement the managed fall protection program.

o         Be knowledgeable of current fall protection regulations, standards, equipment and systems.

o         Advise and provide guidance to managers, employees, and other departments on all managed fall protection program matters.

o         Establish and assign all duties and responsibilities outlined in this standard to trained and qualified individuals.

o         Provide or verify that the personnel have the necessary resources to accomplish their duties and responsibilities.

o         Establish and implement a system to identify, and eliminate or control, new and existing fall hazards.

o         Ensure that written procedures for fall protection, rescue and evacuation are developed for every foreseeable fall hazard to which persons are exposed.

o         Develop training programs for all Authorized Persons.

o         Verify that all Authorized Persons are provided with training.

o         Provide participation (personally or other Qualified person) in the investigation of all incidents related to falls from heights, including:

·          Reviewing incident reports;

·          Taking corrective action to eliminate causes;

·          Making necessary reports to management; and

·          Maintaining an incident report system.

o         Measure and evaluate the effectiveness of the managed fall protection program by:

·          Conducting periodic program evaluations, and

·          Making improvements.

Roles and Responsibilities - Qualified Person

§          The Qualified Person shall have:

o         An accredited degree in structural/civil engineering

o         Fall Protection knowledge,

o         Fall Protection training, and

o         Fall Protection experience.

§          The Qualified Person shall:

o         Be in compliance with local, state, and federal building code requirements, OSHA regulations, and ANSI Z359 standards.

o         Practice in their professional area of expertise as required by:

·          Each state professional ethics standards,

·          The National Society of Professional Engineers (NSPE) "Code of Ethics for Engineers", and

·          The American Society of Civil Engineers (ASCE) "Code of Ethics".

§          The Qualified Person shall:

o         Analyze and design all anchor points for structural engineering strength requirements as set by this standard and also be in compliance with local and state building code requirements.

o         Analyze and design all HLL systems for structural engineering strength requirements as set by this standard and also be in compliance with local and state building code requirements.

o         Establish the clearance requirements for each fall protection system selected.

o         Verify the fall protection equipment system is adequate for the specific fall hazard abatement option chosen by the Competent Person.

o         Maintain a safety factor of 2.0 against failure of the structural system for both anchor points and HLL systems.

o         Establish adequate vertical and horizontal clearance requirements for each fall protection system.  Swing fall consideration shall be analyzed and limitation requirements shall be established for each fall protection system.

Roles and Responsibilities - Competent Person

§          The Competent Person shall:

o         Be fully knowledgeable of applicable fall protection regulations, standards, equipment and systems, and mandatory requirements for fall protection equipment and systems used by their Employers.

o         Have the authority to stop the work immediately if it is determined that it is unsafe.

§          The Competent Person shall:

o         Conduct a fall hazard survey to identify fall hazards.

o         Establish the clearance requirements for each fall protection system.

o         Verify that available clearance is adequate before allowing persons to work at heights.

o         Supervise, for work restraint, travel restraint, work positioning, and non-certified fall arrest anchorages:

·          Installation,

·          Use, and

·          Inspection.

o         Verify that fall protection systems have been installed and inspected in compliance with this standard and all applicable federal, state, and local regulations.

o         Identify, evaluate, and impose limits on the workplace activities to control fall hazard exposures and swing falls.

o         Communicate limits to all Authorized Persons who utilize the fall protection systems.

o         Prepare, update, and approve written fall protection, rescue, and evacuation procedures.

o         Specify that written fall protection, rescue, and evacuation procedures include:

·          Anchorage locations,

·          Connecting means,

·          Body supports, and

·          Other required fall protection equipment.

o         Review the written procedures as workplace activities change for needed additional practices, procedures, or training.

o         Verify that Authorized Persons:

o         Are trained and Authorized,

o         Have had the fall protection, rescue, and evacuation procedures reviewed with them before work begins.

o         Provide or ensure that a prompt rescue can be accomplished with the rescue and evacuation procedures.

o         Participate in the investigation of all incidents related to falls from heights.

o         Immediately remove from service all damaged equipment or components, or equipment or components that have experienced a free-fall arrest personal fall arrest.

o         Inspect for damage and follow the manufacturer's instructions for damaged equipment and equipment that has experienced a free-fall arrest.

o         Periodically inspect all fall protection equipment as per the manufacturer's requirements and applicable regulations.

Roles and Responsibilities - Authorized Person

§          The Authorized Person shall:

o         Follow the instructions of the program administrator and the Competent Person regarding the use of fall protection systems.

o         Understand and adhere to the fall protection procedures.

o         Notify the Competent Person of unsafe or hazardous conditions or acts that may cause injury before proceeding with workplace activities.

o         Correctly use, inspect, maintain, store, and care for fall protection equipment and systems.

o         Inspect any fall protection equipment, prior to use, for defects or damage.

o         Notify the Competent Person of defects and damage found from inspections.

o         Not use equipment with defects or damage.

Roles and Responsibilities - Competent Rescuer

§          The Competent Rescuer shall:

o         Be knowledgeable of current fall protection and fall rescue regulations, standards, equipment, and systems.

o         Know the general hazards associated with rescue and the procedures necessary to mitigate the hazards.

o         Anticipate the foreseeable potential for rescue from the written fall protection procedures and plan accordingly.

§          The Competent Rescuer shall:

o         Prepare, update, review, and approve the fall rescue and evacuation procedures.

o         Identify the resources necessary to conduct a safe and effective rescue.

o         Verify that those resources are available for a prompt rescue.

o         Verify that all Authorized Rescuers have been properly trained and are proficient at performing facility specific rescue.

o         Develop and implement, for rescue equipment, proper procedures for:

·          Selection,

·          Construction,

·          Assembly,

·          Use,

·          Storage,

·          Maintenance, and

·          Protection against damage by workplace conditions.

o         Verify that the rescue equipment markings, instructions, and use are consistent with this standard and with the rescue and evacuation procedures.

o         Verify that response procedures and rescue performance are reviewed:

·          At least twice annually,

·          By a Competent Person, a Qualified Person, a rescue trainer or a Competent Rescuer,

·          To assess which rescue procedures are actually followed during rescue operations.

·          To determine whether further training may be necessary and to identify areas in which rescue performance may be adequate but can be improved.

Roles and Responsibilities - Authorized Rescuer

§          The Authorized Rescuer shall be knowledgeable:

o         In the selection, use, storage, and care of all equipment necessary to perform a rescue at their facility.

o         Of current hazards that may endanger the rescuer during rescue operations.

§          The Authorized Rescuer shall:

o         Consider his or her safety first in determining the method for performing the rescue.

o         Verify that a rescue and evacuation procedure is in place for all work where persons are exposed to fall hazards.

o         Review and approve that procedure prior to work beginning.

o         Inspect the rescue equipment according to procedures developed by the Competent Rescuer.

o         Verify that all rescue equipment is properly protected in proper working condition, and safe for rescue use.

Roles and Responsibilities - Qualified Person Trainer

§          The Qualified Person Trainer shall:

o         Be knowledgeable of applicable fall protection regulations, standards, equipment and systems.

o         Evaluate the fall hazards and the equipment used in the industry they are instructing. 

o         Customize the training to that industry.

o         Any person conducting Qualified Person training must meet the definition of a Qualified Person.

§          The Competent rescue trainer shall be knowledgeable of:

o         Applicable fall protection regulations,

o         Standards,

o         Equipment, and

o         Systems. 

§          The Competent Rescuer trainer shall:

o         Evaluate the circumstances and conditions under which rescue may be required,

o         Provide training that is appropriate to those circumstances and conditions, and

o         Evaluate the rescue knowledge and rescue skills of Authorized rescue trainees.
