§          All incidents shall:

o         Be reported to the Competent Person or the Qualified Person, and

o         Be investigated promptly.

§          An incident investigation shall consider all factors that contributed to the event including, but not limited to a review of:

o         Policies,

o         Procedures, Training,

o         The fall hazard survey,

o         Equipment and related systems, and

o         General communication.

§          After the incident, all equipment associated with a fall shall be immediately removed from service until the investigation is complete.  A Competent Person shall inspect for damage and determine whether the equipment is suitable for service.

§          Incident investigations shall be retained on file for at least 5 years or longer if mandated by a legislative authority.
