§
The
Managed Fall Protection Program shall be evaluated by the program
Administrator. §
The
program shall be evaluated: o
Periodically
at least every two years, or o
When
Authorized Persons or Competent Persons identify deficiencies, or o
When
there are incidents involving injuries, property damage, or "near
misses". §
The
documented Program evaluation shall identify for each element: o
Program
strengths, o
Program
deficiencies, o
Recommendations
for improvement. §
A
documented plan of action shall: o
Be
developed, o
Identify
what changes will be made, o
Identify
who is responsible for making each change, o
Identify
when the change will be completed. §
Program
evaluations shall be retained on file for at least 5 years,
or longer if mandated by a legislative authority. |

The
program evaluation shall review: o
The
written policy statement and its application in the workplace. o
That
duties and responsibilities have been appropriately assigned. o
That
the education and training for the above personnel is adequate to perform
their duties and responsibilities. o
The
training programs to verify that fall protection training conforms with the
American National Standard Z490.1-2001, Criteria for Accepted Practices in
Safety, Health, and Environmental Training, addresses known hazards and
utilizes equipment currently used by authorized persons. o
The
existing fall hazard survey reports to ensure the existing surveys are
current. o
All
selected fall hazard controls. o
All
anchorage systems. o
The
process for purchasing fall protection equipment/systems to
ensure such equipment/systems meet the requirements of this standard. o
The
inspection and maintenance systems, and storage and care for fall protection
systems. o
The
fall protection procedures, rescue procedures, and evacuation procedures. o
The
insurance claims to identify procedures, work areas, or equipment for an
indication of hazards not properly controlled or eliminated. o
The
performance reviews of Authorized Persons, using the proper equipment and
procedures. o
The
results of rescue and evacuation drills. o
The
fall protection and prevention considerations for new buildings, facilities,
and equipment. o
All
incident investigations as outlined in section 7.0. |
